Background Screening Step-By-Step Instruction Guide
Creative Services, Inc. (CSI) has been selected as the Greater Lawrence Community Action preferred background screening vendor.
Greater Lawrence Community Action Council (GLCAC) strives to maintain a safe, secure, and productive work environment and to protect company assets. GLCAC requires all agencies and contracted service providers to perform background checks on their current and prospective employees who will be performing work for GLCAC. Agencies and Contracted service providers include all vendors providing service under contract to GLCAC whether performed on-site or remotely, within or outside of the US.
The following instructions have been provided to facilitate the setup process:
- Read, complete and execute the User Certification and Service Agreement and the End-User Screening Application.
- Fax executed service agreement and screening application to 508-339-2352, Attn: Client Services
- Upon receipt of the above mentioned documents, your background screening account will be activated within two (2) business days and a welcome package will be emailed to the addresses provided on the End User Screening Application. The welcome package email will include your ordering instructions and your account manager contact information.

Note: Some states require state-specific release forms:
- NH Motor Vehicle Driving Record (if applicant has a NH issued license)
- PA DOT Affidavit (if applicant has a PA issued license)
- If a motor vehicle driving record is required in the State of Pennsylvania, the PA DOT Affidavit must be completed prior to receiving motor vehicle information. The PA DOT Affidavit must be received and approved by the Pennsylvania Department of Transportation and may take up to 3 weeks to process.

FAQ's
What if there is uncontested adverse information in the report and I would like to make an exception request?
In the event an employee or prospective employee does not meet the background check contract requirements, the agency or contractor can complete a Background Check Exception Request Form. Completed forms can be faxed to 978-681-4980 or emailed to rrozzi@glcac.org and jguay@glcac.org
What if my company is being audited?
In the event an audit is being conducted on your company, complete the GLCAC Contractor Audit Form. Completed forms can be faxed to 508-339-2352 or emailed to clientservices@creativeservices.com
What if a background check has already been conducted on a current employee?
The term “Background Check” can be ambiguous. In an effort to ensure safety and consistency, Greater Lawrence Community Action requires that all current and prospective Agency and Contractor employees are subject to the same risk avoidance practices.
How long does a background check take?
Generally, background checks are completed within an average of five (5) business days, however, in some cases, processing time can be delayed due to situations beyond CSI’s control.
How will I be notified when a background check is complete?
CSI will automatically send an email to the designated representative when background check information is available. All information is sent in a password protected zip file (requires WinZip).
What happens if an Agency or Contractor refuses to conduct background checks?
Non-compliance with the Greater Lawrence Community Action Background Check Policy will result in removal from any contracted work.
How do I know information is safe?
In addition to federal regulations including FCRA, CSI complies with state regulations relating to data protection including the Massachusetts Data Security Law, 201 CMR 17.00, which sets the most stringent standard for the protection of personal information in the United States. What do I do if there is adverse information contained within the report?
The Federal Fair Credit Reporting Act (FCRA) requires that the consumer be given an opportunity to review the information BEFORE any adverse action is taken. Provide a copy of the consumer report and the FCRA Summary of Rights with the pre-adverse action sample letter to the consumer. All disputes should be directed to:

What if there is a mistake in the report?
Although CSI makes every reasonable effort to provide accurate results, CSI cannot be either an insurer or a guarantor of the accuracy of the information reported. In accordance with the Federal Fair Credit Reporting Act (FCRA), all consumer accuracy disputes should be directed to CSI’s Compliance Department.

In accordance with federal regulations, CSI will promptly initiate a re-investigation at no cost to the client or consumer. Results of reinvestigations will be directed to the authorized representative as well as the consumer.
