CVS requires all contracted service providers to perform background checks on their employees who will be performing work for CVS. The following documents have been provided to facilitate the setup process.
- User Certification and Master Service Agreement
- End-User Screening Application
- Payment Authorization Form
- Appendix A – Services Attachment
- Appendix C – Notice to Users of Consumer Reports
The following instructions have been provided to facilitate the setup process:
- Read, complete and execute the User Certification and Master Service Agreement and End-User Screening Application.
- Execute Payment Authorization Form (Please note: requests cannot be processed without the Payment Authorization Form or a Purchase Order with specific billing and payment instructions).
- Provide section of contract that specifies background screening requirements and adjudication standards.
- Fax/email entire package to 508-339-2352/ clientsuccess@creativeservices.com, Attn: Client Success.
Upon receipt of the above mentioned documents your background screening account will be activated and you will receive your login credentials.
